American Income Life’s Special Risk Division policyholders may submit activity reports and print out various claims forms online.

Activity Reports

Send us your activity report online. Click ‘Submit Activity Report’ and fill out the form with all required information. Remember to include the activity description, estimated number of people participants, and the policy number. When you have entered your information, click ‘Submit’ at the bottom of the page.

Print your claim form instantly. Instead of waiting to receive a claim form by mail, you can fill it out directly from our website. To print the claim form, just click the link and print out the form. Then fill it out, and either mail or fax the form to the AIL Special Risk Division. Remember to attach all necessary information when you mail the form.
